v9.0 - closed-topic summary guidance and startup patch posture refinement
Summary
This release refines the RULES repository in two core areas:
- closed-topic presentation guidance
- startup patch posture for greenfield / baseline formation
It also includes postflight cleanup of residual metadata and master-changelog drift.

Behavioral changes
Closed-topic presentation
Resolved issues may remain available as reasoning context, but they should not be repeated in active summaries by default once the conversation has moved on. Historical context should be labeled explicitly when still needed.
Startup patch posture
Patch remains valid for existing governed surfaces with real before/after review value, but greenfield startup / baseline formation now defaults to design + changelog + TODO + phase first, with patch normally not required unless explicitly requested or justified by a real review surface.

Summary
- refine startup patch posture so greenfield baseline formation no longer implies patch by default
- add Waves 018-021 for the RULES compact plugin companion, including ephemeral handoff, session-scoped carry-forward, active review trigger, and reference-first review tightening
- replace legacy compact hook scripts with the new session-scoped compact lifecycle scripts
