A very non-exhaustive list of things that can go wrong
These can often spring from collateral damage or other actions, and need to be fixed
Something in the pipes/ducts/electrical grid/network grid fails, causing downstream loss/havoc
If radio coverage isn't up to scratch, orders can be mistaken for others
An experimental biological specimen is on the loose. It's confused and causing damage.
A civilian panics and breaks into an alarmed room
Facility staff member having a minor breakdown, refusing to exit bathroom
Warehouse container breakage, spill of fuel, parts, or other debris
Fire extinguisher identified as not having been filled since last inspection, is empty
Maintenance guy didn't show, elevator/tram still out of order
Because you can't have a disaster game without zombies. Could be slow or fast.
One of the complex AIs has gone off grid, possibly convincing others to come with. Break in and take it offline however you can.
A self-replicating nanobot has come about, either through incompetence or maliciousness. Contain the spread however you can.
A hostile force is occupying and attempting to steal as much as possible before retreating. Move fast, quarantine whatever disasters they've put in your way, and stop them from extracting anything else
Someone opened up a portal to an Alien planet with biology similar to ours - and they're invading. Shut down the portal and secure the facility.
An experimental combat biological specimen is on the loose. It's hostile.
A virus jumps containment and infects nearby personnel.
A cult is ritually sacrificing employees in an insane ritual, and needs to be stopped.
