[POC] Cleanup generated output structure

this is a proof of concept for issue #87

it refactors the SDK generator (bin/main.ts) to output the generated files into a dedicated generated/ subdirectory instead of mixing them into the root output folder

key changes:

• generated client, schema, aliases and submodules are now emitted inside generated/ folder
• small additional cleanup on how index.js and index.d.ts re-export from generated content.

not fully tested across all edge cases yet, but should serve as a starting point for the proposed output structure refactor
