Automatically adds prefix and number to commit messages

Automatically adds prefix and number to commit messages:

• Automatically figures out commit message prefix and number from branch name and github org name
• add -r option to provide custom prefix
• changed -j option to "-n" or "--number"
• added '-a' or '--all' to automatically stage files; works for adding, removing and modifying files.
• updated readme
