Besides other changes, this commit ensures the generated HTML doc for HexDocs.pm will become the main source doc for this Elixir library which leverage on ExDoc features. List of changes: * Use and set latest ex_doc * Update gitignore * Update formatter config * Fix typos * Use common source url * Add source reference * Refactor project config * Add changelog to html doc * Add missing changelog release dates * Fix markdowns * Badges and more badges! * Add license section

Tried to run the test but failed. The maintainer of websockex seems stopped working on the repo.
Have to fork our own version of websockex.
