- alto.in.NsBinding#createDefaultNs now uses XMLConstants.DEFAULT_NS_PREFIX as prefix. - added tests with expected behaviour after fix - com.fasterxml.aalto.in.NsBindingTest - com.fasterxml.aalto.in.FixedNsContextTest
