Create all formats

I've been thinking about it and it's not worth just making an EPUB and telling the reader to go convert it if they want. We might as well just take the extra steps, even though this is slightly inconvenient for us.

It's possible that these artifacts should also be checked into a repo such as in some artifact folder, because even though the GitHub release tab is pretty good I don't super trust it as much as I trust to git itself.

since one of our formats will probably be the "manly" home-printing format, we can use that opportunity to merge the manly branch into master, thus unifying the branches and thereby simplifying the branch story here to the minimum (1).
