Any reason you are referencing the old libs for libfmvoice, libvgm and midilib apart from stability?
I tried with the latest versions of these libs and after some code shuffling, fmtoy runs fine. At least the first two have had useful bugfixes since the linked versions, while midilib is currently not really needed in my opinion anyway as only one line of code is used from it.
