Create an Issue template and PR template for the repo

• Create a ISSUE_TEMPLATE.md file under .github folder
• Create a pull_request_template.md file under .github folder

@timcera created this issue for better collaboration with the community. Currently, there is no PR template and Issue template. Although you have mentioned those things in the contribution guidelines it's good to have issues and PR templates too.
