Bug: Randomly won't display all options in context menu

Love the extension. Thank you so much for it!

Code Spell Checker version: v4.2.6
VSCode version: 1.91.0

Occasionally when I right-click on a word the extension has underlined and want to add it to my dictionary, I only get the following options:

• Create a CSpell Configuration File.
• Create a Custom Dictionary File.

I don't see:

• Add Words to Workspace Settings
• Add Words to User Dictionary
• Add All Spelling Issues to Dictionary
• Ignore Words

Do you mean that the items above in the green do not show up?
