I use pure OpenCode v1.14.21 and OMO 3.17.4, without extra system prompts
applied this plugin, anthropic still reply 400 for my request, even with Claude Code Prompt on
only works when disabled Client Prompt but this will affect OpenCode performance

I have the exact same issue as described. Unfortunately the fix from @CrazyCoder does not work either :(

The fix is still holding for me and works fine after Apr 26 commits.

Yeah sorry, ignore my comment. I did a very smart thing: I wrote a OpenCode plugin, that uses this package to scrub the system prompt. But i guess meridian "ignores" that and sends some kind of default prompt (Client Prompt enabled in settings), that Anthropic blocks...
Anyways, I now use this plugin as it was intended: As a meridian plugin, and it works - Thanks!

Update on this. Two fixes landed:

1. The <env> de-duplication that resolves the 400 was merged after your original report — but I just found and fixed a bug where it wasn't actually stripping the block in the normal flow (the powered-by line was consuming the newline the rule anchored on). Proven live now: the raw prompt 400s with out of extra usage, the scrubbed prompt succeeds.

2. If you're on OhMyOpenCode, its 4.x personas changed shape (new <agent-identity> block, **Sisyphus** instead of \"Sisyphus\") and needed new scrub rules — also in this release.

Both are merged with a live-verified test suite. Please update to the new release when it's on npm and reopen with a prompt dump if anything still 400s. I'll comment here when it publishes.
