Inconsistent inta-doc link resolution in for Hover

Link rewrite for hover requests doesn't work across crate boundaries for some definition types. I haven't experimented much with what combinations do and don't work, I discovered this when trying to add functionality to kak-lsp. Can investigate more if this isn't know behavior.
