Update CHANGELOG.md with full v0.4.0 entry covering rules consolidation (47 → 17), Copilot agent addition, and new governance/maintenance categories. Bump VERSION to 0.4.0. Fix stale rule counts across README, CONTRIBUTING, CLAUDE.md, and capability-levels.md. Note tombstone backfill for post-0.5.0.
