Go to 'v1beta1 appwrapper file'
Scroll down to line 29
See error "appwrapper.mcad.ibm.com"

What Have You Already Tried to Debug the Issue?

I am not sure if this part of the MCAD code is still active, if so the appwrapper reference needs to move to codeflare.dev identifier.
