Throw an error if the database has not been migrated

We recently noticed that if pia-server is started using `lein ring server` with an unmigrated database, it is possible to start a flow and retrieve the run without a problem, even though no records are created in the db (obviously). We need to investigate why errors are being swallowed or are not being thrown.
