Chaining iterators of inexact length

#105 enables chaining of parallel iterators where the length of the first iterator is known. The implementation is more involved when this is unknown. This issue targets enabling chaining in the latter case as well. The fix most probably requires changes in concurrent iterator implementation of chaining.
