Conditional Logic in Elementor Form Display

Describe the Problem

I'm having trouble adding and displaying a conditional logic field selection. Currently this is running on a custom solution, but I would like to implement a form like this on several sites.

The conditional field that I'm attempting to display is connected (sub-selection) to the "Sponsor Bonus". Thus far, I've only been able to get it to display "Sponsor Bonus".

I've attached a short script that assembles the table display.

I've added the plugin "Conditional Logic Emails and Fields and Redirect for Elementor Forms", and this does help with the conditional fields while creating or editing the form. It might also have a feature that would solve this issue, but I'm still on the learning curve.
