Add Terraform Documentation With Sample Configurations

Add documentation on using Terraform to setup infrastructure and provide sample configurations for some of the common deployment architectures.

An example would be to setup the whole OpenSRP infrastructure in dedicated hosts for each application and separate database servers on AWS.
