UI form for launching a DAO

Eventually we should move away from having to use the command line to launch a DAO and create a UI as many people get nervous about using the command line.
Ideally, we could input the parameters Token Name, Symbol, etc into a UI form instead of a .env file.
Also on the wishlist - set other DAO parameters - such as voting period in a UI
