Hello world - Compilation didn't run at the launch of the extension (press F5)

What went wrong?

The problem in this previous issue is still happening: #510

I follow the steps here: https://code.visualstudio.com/api/get-started/your-first-extension

I hit F5 and no "out" directory is created, but a new vscode window is opened and I can attempt to execute my Hello World command.
When I do execute the command I get an error saying the command is not found.

I am on Windows.

Note if I run "tsc --watch" and leave it running, it works. I have to close the extra vscode window, change my code, then hit F5 again, but that is good enough.
Does vscode try to do that and fails for some reason?

same issue, when I debug it doesn't generate /out but when I do what @connorsadler said it generates /out and debugging works! Otherwise getting an error that says .helloworld not found
