Add 1 new surgery types to "surgery_type" fields for MUTO-INTL

This request from MUTO-INTL was communicated to us earlier in September, but not implemented in the dictionary. The followed two new terms will be added to the surgery_type field:

Head and neck surgery (Reference: NCIt C15454)
