Add one optional Donor field - country_of_residence_at_enrollment

Mutographs program has recruited cases from around the world, so they can provide the information pertains to the location of the recruiting center for each case. 
After discussion with the program, we decide to add an optional field - "country_of_residence_at_enrollment" in Donor table to record the information.
