Update layout to use Query Monitor tables

Currently the debug bar output is just output into a query monitor panel. We could improve the appearance of this by putting it into a table or at least styling the list of headings and dropdowns. It's a bit hard to read currently.

I'm going to fix this directly in QM soon. I've got styling overrides for some other Debug Bar add-ons in there already.
