Wish for more fine-grained options #97

• I'd like spaces after commas in declarations, but not in arguments of procedure definition or call, or in array indices. Fixes and new options #99
• I'd like indenting blocks, but not the first level (e.g. if a file contains just a subroutine, I don't need all lines indented). Fixes and new options #99
• I'd lke spaces in if (...) and end foo, but not in write(...) or close(...).

I know one can't please everyonev, but maybe some of these could be considered :)
