Change default org and Authorize org disappear from toolbar and command pallet with latest VSCode extension

Steps To Reproduce

If I update the salesforce.salesforcedx-vscode-core extension component past v64.17.2 say to v65.9.1 I lose the Open Org and Change Default Org from the lower toolbar and the SFDX:Authorize an Org from the VSCode command pallet.

I don't see any errors anywhere those function are just gone

I can't update the CLI, long story, is there a reason 2.114.5 does this?

Hi @hungrypipo, Please run the following and let me know if you still see the issue:

1. Quit VSCode
2. Open a new Command Prompt window
3. cd .vscode/extensions
4. for /d %i in (salesforce.salesforcedx*) do rmdir /s /q "%i"
5. Reopen VSCode
6. Reinstall the Salesforce Extension Pack in VSCode
