Develop plan for issue management and the VMR

Develop a plan for how issues should be managed w.r.t. the VMR. Roughly, the requirements are:

• Individual repositories should not be a dumping ground for large numbers of issues that are outside their scope.
• The issue setup should be such that non-experts should most likely file issues in the right place, or at least a single wrong place.
• Issues that end up in a common location should be triageable into the correct repo at reasonable cost.

The plan should include:

• Layout (where issues go)
• Responsibilities (who is responsible for triage)
• Technology involved (if required).
