Required fields in user-created forms are not enforced

When creating/editing forms, there are options to set each field as required or not required (image 1). These requirements are not being enforced. Form users can submit the form without filling in all required fields. This leads to issues of entries with nothing but an id being created.

service name is set to required	client can create services without names. i've created a few empty ones but only the one with a name shows up here	dropdown menus with the internal type of service still has the ghost entries
