[BUG] Code fences mask first two characters

Initially logged as an Obsidian bug, but may be theme related - I'm unable to duplicate the bug with other themes.

This appears to be related to the 'Fancy Code Blocks' Style Setting, and the first appeared after the latest update to Obsidian v1.9.10

Leading characters reappear after code-block line numbers are removed.

The css for padding-left seems to be ineffective, I copied css from the theme and used custom css to add this and it worked for me :)

  padding-left: 2.8em !important;
}

Or you can wait for this PR to be merged #187
