Custom hostname to worker lacks clarity

What changes are you suggesting?

The current documetation does not make clear that what you actually want to add as a custom route is the external domain, not the fallback. So if you have worker.example.com as the worker and fallback.example.com as the fallback, you shouldn't just add fallback.example.com as a route but also, subdomain.customer.com.

Looks good. On my phone so I can't contribute myself but could the route vanity.customer.com/* in workers routes be documented? So it covers the cases all routes to worker, all routes except one(or multiple), and only specific routes
