Scan Form

When using a mobile device, the application should allow the user to scan a form by capturing a picture with the camera. Once the user takes the picture, the application should display the picture and let the user scan more forms.

Note that the initial version of this workflow will not do anything with the pictures taken. It is just establishing the foundation. Once this work is complete, further development will be done to consume the pictures.
