Add a usage example

I'm sorry but I can't figure out how to use this plugin. I created a basic test.html with 3 HTML titles and added toc: true in the metadata. During generation, docpad is crashing with the following error.

Would you be kind enough to provide an example source page with the necessary metadata, and the generated HTML code ?
