Add full-physics monoblock example.

Add a third MOOSE input file to examples/monoblock/ derived from from the file created in #11 . This full-physics model should have the following features (compared to the intermediate model):

• Inclusion of the fluid region and heat extraction as coupled CFD.
• Non-linear materials with plasticity & visco-plasticity modelled.
• Transient solution, including as much of the monoblock lifetime as possible (i.e. pre-stresses from the manufacturing processes as well as stresses due to operational conditions.)
