I just installed the extension and it works great, although for some reason this is displayed instead of a reasonable icon. This may be related to having two "vscode" entries installed. I use Omnissa Horizon Client for work there's a Visual Studio Code entry it creates for the remote VDI VSCode. This causes me all sorts of problems but is unfortunately something I can't really control.
Any hints about what may be causing this?

I am currently not certain what is causing the issue. I have to do more debugging; the main issue is that I cannot replicate the problem. On all of my machines the icon shows up just fine - even in fresh vms I've spun-up for testing.
I will create a branch that adds debugging to the icon selection logic, it would be really helpful if you or the other user could test this and let me know what is returned. Will let you know when I get this done.
