Disconnect an application as Org Admin

For connected applications show a disconnect button with modal and informative message. Result is a Jira ticket and a corresponding change request is Manage.

• Only Organization admins may request a disconnect request
• When the disconnect is requested, prevent the user to make a new disconnect request
• Jira ticket is created
• Change request in Manage with a removal of the application from the allowed list
