Here is the rev2 version of form issues from Jake. Some of these issues may be resolved. If so, please link the PR that resolved them and strikethrough.

To help with addressing the issues below, please follow these steps:

If you are actively taking on the issue please check the box.
Once your PR to address the issue has been merged in please do strikethrough on the issue and reference your MR link that addresses it.

Issues:

    • Phone number still doesn’t want to populate between forms. This time between Conviction Petition and Motion to Waive Fees. Fix phone number #203
    • Tab button sometimes refreshes page and sets you back on the original case you ever entered.
    • Order on Motion to Waive Fees: Respondent NOT defendant. Petitioners name still not populating. This is resolved but don't see a PR to link
    • When I click justice court for the first time, it always crashes the system. Not seeing this happen. Believe it is resolved
    • On Order on Motion to Waive fees, get rid of auto populating on State of Utah. In fact, just get rid of populating the respondent altogether, no need Fix respondent #204
    • Page 2 of District Court Coversheet is Missing This is resolved but don't see a PR to link
    • Motion to Waive Fees does not populate employer’s address and phone number, job title, or hourly rate or annual salary Populate employer info #209
    • Does not provide subtotal for total monthly gross income
    • Does not provide subtotal for total monthly tax deductions, nor does it calculate after-tax monthly income
    • Does not provide subtotal for monthly expenses
    • Motion to waive populates across Cases, BRILLIANT BEAUTIFUL GOOD JOB! But the I am employed part does not populate across
    • American Fork District Court is populating the address as: Church St. and E Main St.(Approximate Address) 75 East 80 North, Suite 202American Fork, UT 84003-0986. Get rid of everything before 75 East 80 North, Suite 202American Fork, UT 84003-0986. It’s running onto a second line which is actually pushing down some of the pages somehow, so if needed abbreviate to 75 E. 80 N. Ste. 202American Fork, UT 84003-0986 Fix american fork address #208
    • On the Cover Sheet for the District Court, name the file as District Court to keep it distinct from Justice Court Coversheet. This is resolved but don't see a PR to link
      • Add a check in the box on the first page bottom left where it says “This case is exempt from URCP 26”.
    • On the Order to Expunge (Dismissal or Acquittal) remove the Judge’s name from populating automatically at the bottom of the second page below the signature line.Judge’s often sign for other Judge’s if they are out sick, etc. Remove judge name #211
      • Double check other Orders to make sure Judge name is not populating below signature line. Remove judge name #211
    • Petitioner’s name is not populating on the Plaintiff/Petitioner line on the Order to Waive Fees. Not seeing this happen. Believe it is resolved.
    • On the Order to Waive Fees, on page 2 in the box on the top half of the page, the bottom left bracket has a box that says Other (Describe.) Go ahead and insert the words “Certified Copy Fee” but leave the checkbox blank. Add certify copy fee #212
      • Remove the Defendant line auto populating State of Utah, remove the input altogether, unnecessary for them to know the Respondent. Fix respondent #204
    • On each of the Petitions, open up the box wider for the Public Interest Statement. Have 6 lines actually showing up on screen, to encourage the person filling it out to write more than just one line.
    • On the Justice Court coversheet, check the box $135 Expungement Petition
    • Petition to Expunge Drug Possession the check mark for the courts is showing up below the box.
    • Petitioner name is not populating on the Petitioner line on the Order to Expunge Drug Conviction form.
    • This Order also autopops a check mark in “the pleadings and other papers..” this needs to be removed.
    • On South Salt Lake Justice Court, get rid of (2430 South) from populating.
    • Case number not populating on Order to Expunge Drug Conviction
    • Farmington District Court is missing.
    • On the Order to Expunge Charges Never Filed, remove the option to check if it will be resolved by pleadings or hearing.

@Mengqi89 I see an open issue for the Justice Court coversheet. It looks like it's been resolved. If so, I will close it and cross of item 5 above.

@oliviaclyde Thanks so much for creating the list. I believe 5, 7, 8 and 22 are all resolved. I wanted to double check if item 9 is resolved but I am not sure which form it refers to. I recall addressing a similar issue in the past.

Issue 10 and 29 are duplicates. Item 4 does not show up as an issue.

Doing some clean up of our issues list. I have linked and crossed out issues that have been resolved (most with PRs linked)

Item 11 - per Tucker address is correct as is. Resolving issue

Item 36 above, I'm not seeing this issue. The only address I'm seeing populate is "220 Morris Ave #200, Salt Lake City, UT 84115". I checked the JusticeCourtList.json file and the address I referenced is what populates on the form. @Mengqi89 are you seeing this issue? If not, I believe the address was previously corrected and this issue is resolved.

Item 41 above, I'm not seeing this issue. I'm seeing the Farmington District Court populate under Davis County with the address: 800 W State St, Farmington, UT 84025. Are you seeing this as well @Mengqi89 ? If so, then this issues has been corrected and is resolved.
