The checkbox should always say "Expungement Petition". The other fields should be filled out the same as the District Court cover sheet.

The personal information filled in this form should go to "Defendant/Respondent". Correct? I noticed on the district court cover sheet, the information went to "Plaintiff/Petitioner". Just want to make sure of where it needs to go.

Description: "The cover sheet for the paperwork you file if your case is in a Justice Court". Let me know if that's too long.

And actually the person's name should be the petitioner, not the defendant. It's different from the criminal case - in this case, it's a civil action, and they're the ones filing the paperwork.
