Prepare to ship survey questions have confusing form inputs.

Describe the bug
In the sidebar for prepare to ship reviews, the survey questions each have text fields and checkboxes, but there is also a "comments" text field. It isn't clear to me where to put my responses because when I check the checkboxes or type in the text fields which appear next to the questions, the "Post" submit button stays disabled. As a result, I don't trust using the individual checkboxes and inputs and instead just write everything in the "Comments" text field instead. The "privacy" and "testing" ones have these individual inputs, but the other ones don't.

I think that there should be a separate submit button for the individual fields or an explanation which makes me believe that they actually do something.

Steps to reproduce the behavior:

1. Go to a chromestatus entry like this: https://chromestatus.com/feature/6222145025867776
2. Click on "prepare to ship" accordion
3. Click on the "privacy" chip
4. Click on checkboxes and type in text inputs in "survey questions"
5. See that the "post" button is not enabled

I haven't tried it out, but if these individual inputs aren't hooked up to anything, then I think it would be sufficient to just remove them completely.

After you fill in the questionnaire check boxes and text field, click "Request review" at the top of the column.

In the case of the privacy and testing gates, your responses in these widgets may qualify for self-certification, meaning that you will not need to wait for a human review.

Ah that makes sense, I'll do that next time
