Align CSV headers and values with the Results page

Nuri's suggestions:
Ensure the results page headers align with the CSV (CSV is missing mothers maiden name).

CSV example below:
Given Name Surname Record Type Quarter District Volume Page
Josephine Smith BIRTHS Mar-47 Willesden 5f 958
Given name to read first name; quarter should read Registration date; date value is to match the value in the results page
district to read registration district; births to read birth etc.
